Am I Eligible to Submit a Claim?

To be eligible to submit a claim, you must:

• Be 18 years of age or older;
• Be a resident anywhere in Canada;
• Not be an Excluded Person (a person who opted out of the class action or a director or officer of a defendant or its subsidiaries or affiliates); and
• Ella Doucette, either directly or indirectly, harmed your child or you as a parent. If someone else harmed your child, please let us know so that we could let you know of future class actions involving those indivuduals.

PART 2 – Payment Information

PAYMENT: If your claim is approved, you will be paid via Interac e-Transfer to the email address provided in Part 1. Please note you will have 30 days from the date of issue to accept your e-Transfer. The password for the e-transfer will be '2026'.

ALTERNATE PAYMENT OPTION:  Check this box to receive payment by cheque instead of e-Transfer. Please note that $2 will be deducted from your payment to account for the additional cost of issuing a cheque. Your cheque will be mailed to the address provided in Part 1. You will have 6 months from the date of issue to cash your cheque.

 

Verification

To be eligible to submit a claim, you must understand, confirm and agree that:

• The Settlement Administrator may contact me for more information about my claim, including requesting appropriate forms of ID. Please note failure to agree to this may result in your claim being disqualified.
• I have provided my legal name, current residential address and correct date of birth in Part 1 above.
• I am at least 18 years old.
• I resided in Canada (outside of Quebec) on December 31, 2021.
• I purchased Packaged Bread for my personal use in the period between January 1, 2001 and December 31, 2021.

  I understand, confirm and agree this information is true.

 

• The amount depends on how many people submit claims.

If there is money left after the calculation of the initial compensation:

• A supplemental compensation may be made to all approved claimants (including those who got a Loblaw Card under the Loblaw Card Program in 2018 and those who didn’t).
• This supplemental compensation will be calculated pro rata based on how much money is left.

 

Important Notes

• The initial and supplemental compensation will be combined and paid as a single Interac e-Transfer or cheque.
• No payment will be made if the amount is less than $5.

 

Consent To Retain Information
The Packaged Bread class actions are continuing against other defendants and there may be future additional distribution of monies from additional settlement(s) or judgment(s).
